Call Transdev or formerly known as Intelliride then ask Afro Care Transportation as your preferred provider. Then make sure that you have your doctor/counselor will fill out the level of service form from the Intelliride website with the link below and download it. Your doctor/counselor will fill out the form completely and fax or email it to Intelliride at the contact info provided on the form. Please remind your doctor to put ‘Afro Care Transportation’ in the ‘Additional Comments’ field.
